Dr Melfi - the psychiatrist who Tony Soprano began consulting for the panic attacks that dog him - is in many ways closer to him than many of his confidantes, certainly his wife. In her relationship with him, she becomes the (presumably sometimes unwilling) participant in his violent and carefully-plotted acts. The understandable stress of dealing with a mob boss as one of your most frequent patients understandably has its toll on the quiet, insightful and intelligent Dr Melfi, and she begins to deal with it in different ways - firstly by drinking, despite her ongoing struggle with alcoholism,, then by attending a psychiatrist herself, who prescribes various types of medication for her. During the seasons, the sometimes incredibly traumatic acts that she has to endure - including a horrifying assault and rape in a stairwell - leads her to sympathise with Tony's violent ways of dealing with people, blurring the boundary between the image we have of her as the moral "wailing wall" for Tony to air his fears and nightmares to, and making her seem complicit in the violence he describes to her.
